Heart failure is a disease often defined as a clinical syndrome characterized by signs, symptoms, and structural and functional abnormalities of the heart. It has a high mortality and morbidity rate worldwide. One of the risk factors associated with heart failure is Body Mass Index (BMI). This study aimed to determine the relationship between BMI and the incidence of heart failure, as well as to identify the ideal BMI cut-off point as a predictive determinant of heart failure. A cross-sectional study was conducted on 213 heart failure patients using electronic medical records at RSUD Prof. Dr. Margono Soekarjo Purwokerto. BMI was categorized into underweight, normal, overweight, and obesity grades I and II. Heart failure was assessed using echocardiography based on left ventricular ejection fraction. The majority of patients were men with an average age of 65 years. The most common BMI category was normal (31%), while the predominant ejection fraction category was HFrEF (44.1%). Statistical analysis showed no significant relationship between BMI and cardiac ejection fraction (p = 0.128). ROC analysis indicated low discriminatory ability of BMI (AUC = 0.507; p = 0.858). Therefore, BMI cannot be used as a sole predictor of ejection fraction in heart failure patients.
