Pulmonary tuberculosis is a contagious infectious disease caused by Mycobacterium tuberculosis and remains one of the leading causes of death worldwide, including in Indonesia. Long-term tuberculosis treatment often results in various problems, such as drug resistance, poor patient adherence, lung tissue damage, and post-infection pulmonary fibrosis. Therefore, innovative natural-based therapeutic approaches are needed to improve treatment effectiveness while minimizing complications. This study aims to examine the potential of sulforaphane (SFN), a phytochemical compound derived from broccoli microgreens (Brassica oleracea), as an immunomodulatory and anti-pulmonary fibrosis agent through nanoencapsulation technology. This study employed a narrative literature review method using scientific databases, including PubMed, Scopus, ScienceDirect, and Google Scholar, covering publications from 2018–2024. Data were analyzed using critical appraisal techniques to systematically evaluate and synthesize previous findings. The review demonstrated that SFN exhibits antioxidant activity by activating the NRF2 signaling pathway, thereby reducing reactive oxygen species (ROS) production, inhibiting NLRP3 inflammasome activation, and preventing macrophage pyroptosis induced by M.
