The visual system plays an important role in daily activities, with cataracts defined as lens opacity that reduces visual acuity and functional ability in the elderly. Senile cataracts increase with age and are a leading cause of visual impairment, including in Indonesia. Mature senile cataracts cause severe impairment that affects daily activities and quality of life. To explore the functional ability of older adults in daily activities before and after mature senile cataract surgery. This study used a qualitative approach with a case study design through in-depth interviews with 6–8 elderly participants (until saturation point) and triangulation with caregivers. Data were analyzed thematically, including data reduction, data display, and conclusion drawing. The study identified three main themes: visual function impairment before cataract surgery, the impact of cataracts on daily activities, changes in activity ability after cataract surgery. Mature senile cataract surgery has a positive impact on the functional ability of the elderly in activities of daily living (ADL). Before surgery, visual impairment led to limitations, anxiety, decreased self-confidence, and dependence, although adaptive efforts were still observed. After surgery, improved visual function enhanced functional ability, safety, self-confidence, quality of life, and social participation. Thus, cataract surgery not only improves vision but also provides psychological and social benefits for the elderly.
