Abstract

Low back pain (LBP) is a common musculoskeletal problem and a leading cause of disability, impacting quality of life and psychosocial well-being. Pain perception is influenced by physiological and psychosocial factors, including blood pressure through the mechanism of hypertension-associated hypalgesia. This study aimed to analyze the relationship between blood pressure and pain severity in patients with low back pain. A cross-sectional study was conducted on LBP patients at the neurology clinic at KRT Setjonegoro Hospital, Wonosobo. Blood pressure was categorized as normal, high normal, stage 1, and stage 2 hypertension. Pain severity was measured using a Visual Analogue Scale (VAS). Pain distribution was presented as median, IQR, minimum and maximum values, and tested using Kruskal–Wallis (p-value and effect size). The majority of LBP patients were women aged 40 years. The median VAS score for all blood pressure categories was 4 (moderate). Pain was most variable in normal blood pressurand most homogeneous in stage 2 hypertension (IQR 4–4), with lower pain levels compared to normal patients. The Kruskal–Wallis test showed no significant difference in pain between categories.
