Intrauterine fetal death (IUFD) remains one of the leading causes of perinatal mortality and continues to be a significant maternal and fetal health problem. Gestational diabetes mellitus (GDM) has been recognized as a contributing factor to adverse pregnancy outcomes, including intrauterine fetal death, through mechanisms involving maternal hyperglycemia, placental dysfunction, and fetal hypoxia. This study aimed to analyze the association between gestational diabetes mellitus and the occurrence of intrauterine fetal death (IUFD) at KH. Abdurrahman Syamsuri Hospital, Lamongan. This quantitative analytic observational study employed a retrospective case-control design involving 70 pregnant women, consisting of 35 cases (IUFD) and 35 controls (non-IUFD), with a case-to-control ratio of 1:1. Data were obtained from daily obstetric patient records from January 2023 to April 2026. The case group was selected using total sampling, while the control group was selected using simple random sampling. Data were analyzed using Fisher’s Exact Test and Odds Ratio (OR). The results showed that GDM was identified in 6 of 35 cases (17.1%), whereas no cases of GDM were found among the 35 controls (0%). Fisher’s Exact Test revealed a statistically significant association between gestational diabetes mellitus and IUFD (p = 0.025). The OR of 15.64 (95% CI: 0.85-289.38) indicated that pregnant women with GDM had 15.64 times higher odds of experiencing IUFD compared with those without GDM. In conclusion, gestational diabetes mellitus is associated with increased odds of intrauterine fetal death at KH. Abdurrahman Syamsuri Hospital, Lamongan.
