Abstract

Growth monitoring through the electronic nutrition surveillance system (e-PPGBM) is a crucial early detection instrument for stunting prevention. However, high community participation in primary care does not always reflect comprehensive nutritional outcomes. This study aimed to evaluate growth monitoring indicators for children under five, map the spatial risk of growth faltering, and analyze maternal-neonatal outcomes in the working area of Puskesmas Bebesen, Central Aceh Regency, in December 2025. This cross-sectional descriptive study applied a retrospective secondary data review involving 2,298 children under five and 61 pregnant women. Data were extracted from the e-PPGBM database and midwifery cohort registers (KIA). Spatial descriptive analysis classified villages into three risk zones based on N/D ratios. Results showed that the D/S ratio reached 100%; however, the prevalence of growth faltering was 14.19% (326 children). Spatial mapping identified six high-risk villages (Zona Merah, N/D <70%). Maternal anemia prevalence was notably low (1.63%), while one case of Low Birth Weight (LBW) was identified among 17 documented live births. High primary care accessibility does not guarantee optimal child growth quality. Targeted micro-interventions in high-risk zones and strengthened continuum of midwifery care are urgently required to break the stunting cycle.
