Ocular trauma refers to any injury affecting the structure of the eye. Chemical ocular injury is considered an ophthalmologic emergency because it may cause severe ocular morbidity and even blindness. Occupational exposure to pesticides can occur during storage, mixing, loading, spraying, and cleaning of pesticide application equipment. Exposure to organophosphate pesticides often results from inadequate safety practices and improper use of personal protective equipment (PPE). This case report aims to evaluate the prevention of pesticide-related ocular chemical injury through health education and a family medicine approach at Central Jakarta Primary Health Center. A 23-year-old male patient presented with redness and watery discharge in the left eye for one week and was diagnosed with ocular chemical injury due to pesticide exposure. Comprehensive management included education on ocular chemical injury, correct PPE use, personal hygiene, treatment adherence, prevention of recurrent exposure, early management after chemical exposure, nutritional optimization, and family-based support. The intervention improved the patient’s understanding, encouraged behavioral change, and reduced recurrence risk. A holistic family medicine approach focusing on primary prevention proved effective in strengthening adherence and preventing repeated exposure. This case highlights the importance of integrating promotive and preventive strategies into primary health care.
